As part of any Work Experience or Structured Workplace Learning (SWL) students are required to have the appropriate forms signed and ready to go before commencing their placements. Forms can be downloaded via the following link:
Work Placement ASSISTANCE Form
This form is for any student undertaking a Structured Workplace Learning (SWL) placement as part of their VET course in Victoria. This includes VET in Schools and VCAL students. This form is also for any student undertaking Work Experience at a Victorian School. If the student is under the age of 15, please contact the Workplace Learning Coordinator at Youth Connect at [email address=”[email protected]”] as an alternative form is required.
